For patients:

To schedule an appointment at either of our facilities, please call (804) 237-6666. 

For referring providers:

To order a breast imaging study for your patient, please complete the Breast Imaging Request Form [PDF] and fax to 804-237-6616. Your patient will be called to schedule a convenient date and time. For prompt patient service, we are often able to accommodate same-day ordering requests by calling (804) 237-6666.
